How to Access Us

Thank you for your interest in Astralis Stroke Occupational Therapy. It assists us in processing your request if you, a family member or health care provider can complete and send the below referral form with your enquiry. 

We specialise in delivering Occupational Therapy services to adults in the community who have experienced a stroke, acquired brain injury and other related neurological conditions. We visit patients in their homes within a 30km radius of Midland or extending in to the western wheatlbelt region. Our team can assist you in advising of your proximity. 

We can support private, self-managed or plan-managed NDIS participants, GP managed Medicare plans, and are a registered provider with the Insurance Commission of Western Australia (ICWA).
